Introduction

Avon & Wiltshire Mental Health Partnership NHS Trust (AWP) provides mental health services to a population of 1.8 million people across Bath and North East Somerset, Bristol, North Somerset, South Gloucestershire, Swindon, and Wiltshire. AWP offers a wide range of services including inpatient, community, and specialist mental health care, as well as services for individuals with learning disabilities.

The context

AWP embarked on a Service Re-design initiative for the fiscal year 2023/24, focusing on delivering safe and sustainable services. This initiative aimed to address the critical need for service transformation in response to evolving population demands, ensuring consistent quality care across the AWP geography, and optimising resource management. The principal program involved a comprehensive patient journey mapping exercise to understand and outline the requirements for future service provision.

The solution

To meet these ambitious goals, AWP commissioned the development of a detailed patient journey-based mapping exercise, led by an interim placement specialising in service redesign. The discovery phase involved several key activities:
